Clinical Cognitive Assessment System

Clinician User Manual

1. Purpose

This system is designed to assist clinicians in conducting structured cognitive assessments. It enables accurate testing, automatic scoring, and long-term result tracking.

2. Login

1. Open the system in your browser

2. Enter your username and password

3. Click Login

First-time users will receive an email to set their password.

3. Creating a Patient

Before starting a test, register the patient:

4. Starting an Assessment

Locate the patient in the list and click Take Test. The system will begin the assessment immediately.

5. Assessment Tests

5.1 SDMT (Symbol Matching)

The patient matches symbols to numbers using a reference key.

5.2 Episodic Memory

Patient recalls previously shown information.

5.3 Recognition

Patient identifies previously seen items.

5.4 Executive Function

Measures reasoning and problem-solving ability.

5.5 Attention

Measures concentration and focus.

6. During Testing

Do not interrupt the patient during timed tests.

7. Scoring System

Score Interpretation
85% and above Normal
70–84% Mild Impairment
50–69% Moderate Impairment
Below 50% Severe Impairment

The final score is the average of all five tests.

Click Export Assessments to download results as a CSV file.

10. Re-Assessment

To test a patient again, create a new patient entry. Previous results remain saved permanently.
